Medical History Office
The application and removal of the certified copy of medical records, the clinical and other health records, can be made:
- by the person to whom teh information relates
- by a delegate persone, by proxy in writing and photocopy of the identity of the delegator
- by successors in title in accordance with the current law
Any documentation is delivered in a sealed envelope in ways that ensure compliance with teh provisions on confidentiality as provided by law on privacy (Legislative Decree No. 196 of 30/06/2003)
